8. AUTHORIZATON AND DECLARATION

I authorize Zhejiang University - The Hong Kong Polytechnic University International Executive Development Center to use my data from my application form for the following purposes:
i) processing relevant to the 2007 - 2008 admissions to postgraduate programmes;
ii) for the University to extract from its records and for relevant institutions to provide the University with information about my studies;
iii) identifying multiple applications/registration, and checking records of my studies as a student in institutions in Hong Kong and elsewhere;
iv) transferring to the student record system upon successful outcome of my application; this will later on form the graduate record which may be used in support of alumni activities.
I declare that the information given in support of this application is accurate and complete, and understand that any misrepresentation will lead to disqualification of my application for admission and resulted registration in the University. Any fees paid will not be refunded.

本人授权浙江大学－香港理工大学国际企业培训中心使用本人申请表上的资料作以下的用途:
i) 处理/甄选本人2007 - 2008年度的入学申请；
ii) 查阅本人于香港理工大学所有修读记录，包括学业成绩，及向其它有关院校索取本人的修业成绩，作为入学甄选用途；
iii) 确认是否多重申请或双重学藉及查询本人于其它院校或机构内的修读记录;
iv) 如获录取，本人的资料会转为学生记录;
毕业后，资料将成为毕业生记录及用于旧生会活动上。本人谨此声明，此表格内所填报各事项，均属真实无讹，如有任何虚报，则本人的申请及注册资格均会被取消。所缴费用，概不退还。
